Different Methods To Report Compliance Incidents

  1. Telephone - You can report incidents by telephone either calling the Anonymous Compliance  Hotline number at 412.380.8025 or toll free at 1.877.766.3844 and leaving a detailed message. You can disclose your name or you can remain anonymous. You can also call the Compliance Officer directly at 412.858.4000, extension 2228 or 2650.
  2. Email- You can email the Compliance Officer directly with any issues you may have. However, this method will not be anonymous.
  3. Incident Reporting Form- You can print out the linked Compliance Incident Reporting form (above), complete it to the best of your knowledge, and mail it to the Compliance Officer (see #5 below). You may remain anonymous or you can provide your name if you want to know the outcome of the investigation.
  4. In Person- You can always request to speak to the Compliance Officer in person. E-mail or call 412.858.4000 extension 2228 or 2650 to set up an appointment.
  5. Mail- You can mail a report, note or letter to the Compliance Officer via Unison Interoffice mail or regular mail to 1001 Brinton Road, Pittsburgh, PA 15221.
  6. On-Line Anonymous Compliance Reporting Tool - You can make a report anonymously via the Online Reporting Tool that can be accessed by Unison employees at Compliance Reporting on the Compliance Department intranet page. The instructions for use are on the Reporting Tool.

Anonymous Reporting and Confidentiality

The Anonymous Compliance Reporting Hotline, Online Reporting Tool, and the Mail reporting options ensure avenues for anonymously reporting compliance incidents. All reports submitted will be kept confidential to the extent possible (which has no effect on anonymity). All communications to or from the Compliance Officer in connection with a compliance report may be protected by the attorney-client privilege, which belongs to Unison. Unless informed otherwise by the Compliance Officer, Unison’s attorneys, whether employed or retained by Unison, do not represent the personal legal interests of Unison Team Members. Additionally, there is no attorney-client privilege between the Compliance Officer or other attorney employed or retained by Unison and either a Team Member filing a compliance report or a Team Member interviewed during a compliance investigation.

No Retaliation

No Team Member shall be disciplined or suffer any adverse employment related action or other retaliation solely for making a compliance report or complaint in good faith.
